There is a story that when Hagoan Rav Yaakov ZTVK’L was sick and was in need of a Mishebayrech, however the people could not recall Rav Yakov’s mothers name.
Hagoan Rav Moshe ZTVK’L, who was at that minyan told the people just say Rav Yaakov, in the Mishebeyrach and Hashem will now who it is.
We could probably apply the same lesson to our situation, say Rav Zundel (Shlita).

1) The name is Zundel ben TOBA (not TOVA) Faiga
2) When making a misheberach you do not put “Rav” in front of the person’s name
3) When saying Tehillim, you should try to know the name (ploni ben plonis) when you are saying it
4) When you don’t know the person’s mothers name you say “ploni” ben SARAH (for Sarah Imenu)
