The Gemara Brochos 54b made it up
מאי מברך אמר רב יהודה ברוך גומל חסדים טובים

They were saying בא”י אמ”ה הגומל חסדים טובים לחייבים שגמלני כל טוב.
I was also wondering about it. Then a friend of mine (Y.) directed me to ליקוטי מהרי”ח, where he mentions such a nusach.
