The text of the tombstone was written by the family, according to wording formulated by Rav Chaim’s father, the Steipler.

The article clearly says the wording were based on what the Steipler ZTZ”L formulated. That would include, like by many Gedolim, NOT giving any superlative titles.
The Steipler wasn’t known during his lifetime as the Sar Hatorah but Rav Kanyevski was universally known so . .
